MCC analyzes HRA, claim and prescription data to identify and stratify those members with chronic conditions. Individuals can also self-nominate into the DM program via a health survey. Members are educated by program nurses, sent information regarding their conditions, and referred to a variety of online resources specific to the condition(s) they exhibit.

MCC, Inc. helps employers save money. On the average, for every dollar spent for medical large case management, MCC, Inc. will save its client eight dollars. The program includes registered nurses who have received the designation of  "Certified Wellness Coach" from the University of Wisconsin Milwaukee College of Health Sciences, and a State of the Art, internet based, wellness software program used as a tool to benchmark and identify health risk categories.
